Parent/Guardian:

The Lawrence County Health Department will be at the Aurora High School Media Center on Tuesday, October 29, 2019 from 3:30 p.m.- 6:30 p.m. to provide a flu shot clinic. All students enrolled in the Aurora R-VIII School District and their family members in need of a flu shot can participate. This shot is not required, but is highly recommended.

The flu shot for students will be $5.00 (cash or check made out to LCHD) unless you have active Medicaid. You MUST bring your current Medicaid card in order to cover the cost of the flu shot.

Adults needing a flu shot MUST also bring their insurance card. If your insurance plan is an accepted plan, the cost will be covered. If it is not, the cost will be $30.00 (cash or check made out to LCHD). You may call the Lawrence County Health Department at 417-466-2201 with any questions regarding your insurance plan.

Please call the High School at 678-3355, if you have any other questions.

Thank you,

Kala Brown RN BSN
